What are DOT physicals?

The Department of Transportation (DOT) requires all commercial drivers to have periodic physical exams, commonly referred to as DOT physicals. These exams determine whether a driver is healthy enough to operate large vehicles on roadways shared with other drivers. 

If any of the following apply, you need to have a DOT physical before beginning a new position or after 24 months since your last DOT physical:

  • You drive a vehicle that carries more than 15 people (even if unpaid)
  • You get paid to drive a vehicle that carries more than eight people
  • You transport hazardous material within your vehicle
  • You drive a vehicle on the interstate weighing in at more than 1,000 pounds

If you need a DOT physical, your employer will let you know as part of the hiring or HR process. 

What kinds of things get checked in DOT physicals?

Many aspects of a DOT physical are similar to what you expect during your annual physical exams.  

Expect a quick check of your vital signs in addition to the following:

  • Vision and hearing
  • Back and spine
  • Abdomen
  • Lungs and chest
  • Genital and urinary system
  • Nervous system
  • Joints and extremities
  • Gait

These are just a few areas of focus within a DOT exam. The process is fairly streamlined, and your entire visit takes less than an hour. Urinalysis is often part of the exam to screen for drug use. 

What happens if I fail my DOT physical?

It’s frustrating and disappointing to learn there’s a problem with your DOT physical. That’s especially true if you need your results to meet employment requirements. 

Failing a DOT physical means your practitioner detected at least one health issue of concern. In some cases, these exams are the first indication that you have a chronic health condition or other medical problem.
